JOB TITLE: Support Specialist, In-Home SUPERVISED BY: This position reports to the Program Director, Residential or his/her designed, and will adhere to all policies and procedures of St. John’s Community Services. Job Summary: Provides support to individuals with disabilities in their natural homes to develop and/or maintain skills that assist the individuals to be self-sustaining in that environment. Skills and supports provided should enhance the quality of the individual’s life and support the person in performing his/her own personal care, household tasks, daily routines and community integration activities. Essential Functions: + Provide training and support in nutrition, personal hygiene, health care, appropriate dress, communication, interpersonal relationships, home management and use of leisure time. + Accompany individual to and facilitate participation in visits for medical care, therapies, personal shopping, recreation and other community activities. + Provide training or assistance in meal preparation, laundry, housekeeping, and simple home repair. + Provide training and support in the areas of social, emotional physical and intellectual development. + Support the individual in working on the goals they have selected for themselves. + Document the specific support provided to the individual per the Individual Service Plan (ISP) and all progress made toward those identified goals. + Maintain and submit all required documentation (progress notes, incident reports, etc.) on specified due date. + Attend and participate in all relevant agency events, workshops, training and meetings; and + Performs other job related duties as assigned. Qualifications Required Knowledge, Skills, and Abilities: + Ability to gain working knowledge of St. John’s Community Services policies and procedures; + Ability to act as a representative of St. John’s Community Services to the public; + Ability to effectively communicate and coordinate with other staff members and individuals from diverse backgrounds; + Ability to pass a criminal background check; + Knowledge of acceptable food preparation and safety practices and procedures; and + Skilled in the operation of relevant computer systems, including hardware and software, and office machines. Education and Experience: + High school diploma or G.E.D. required + Two or more years related experience working with persons with disabilities preferred; experience in community based programs a plus; + Must meet current SJCS and funding jurisdiction requirements for criminal background/records check. + Must meet current funding jurisdiction requirements for valid health certificate. + Valid state-issued driver’s license with current driving record in good standing required.
