For example: + Mon – Fri: 6pm to 8am = $4.00 Shift Differential + Sat & Sun: 6pm to 8am = $5.50 Shift Differential + Sat & Sun: 8am to 6pm = $4.00 Shift Differential **Summary of Position Purpose and Function:** The Patient Care Coordinator ensures effective, client-centered management of all referrals for assigned service. The Patient Care Coordinator serves as the primary point of contact for referred clients, managing their hospital experience from initial referral through conclusion of service.
