Virtual Certified Diabetes Educator / Virtual Patient Navigator – East – Remote

We are excited to announce that we are currently looking for a Virtual Patient Navigator to join our team.  The Virtual Patient Navigator will provide education about specific diabetes treatments, and resources to help patients better begin and manage their diabetes and resources associated with their prescribed medication. Virtual Patient Navigators are responsible for participating in one-on-one communications with enrolled patients by telephone, video platforms, email and SMS strictly following the guidelines of the program. Since the program is strictly educational based, Virtual Patient Navigators do not provide medical advice or work clinically within this role.

Required and Preferred Qualifications: 

Bachelor's degree and/or BSN Required3+ year as a Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES) requiredComplete understanding and application of principles, practices, and standards in diabetes care directly with patients requiredRegistered Nurse, Registered Dietitian, Physician Assistant, Nurse Practitioner, Pharmacist, or other relevant certifications that are Healthcare related is preferredContact Center Experience using a telephony platform is strongly preferredMotivational Interviewing experience preferred

Must work from a dedicated workspace in their primary home residenceExperience with direct patient interaction and support for persistence and complianceDemonstrated time management skills; planning and prioritization skills; ability to multi-task and maintain prioritization of key projects and deadlinesDemonstrated effective presentation skills; excellent interpersonal (written/verbal/virtual) communication skillsDemonstrated ability to work independently fast-paced work environmentDemonstrated ability to build relationships with customers and cross-functional partnersFully competent in MS Office (Teams, Word, Excel, PowerPoint, SharePoint)Excellent documentation accuracyDrive and enthusiasm for supporting patientsExcellent listening and problem-solving skillsBilingual (i.e., fluency in Spanish and English) is desirable

The potential base pay range for this role is $100,000-$115,000 annually. The actual base pay offered may vary based on a number of factors including job-related qualifications such as knowledge, skills, education, and experience; location; and/or schedule (full or part-time). Dependent on the position offered, incentive plans, bonuses, and/or other forms of compensation may be offered, in addition to a range of health and welfare and/or other benefits.
