Responsibilities
The Virtual Registered Nurse (RN) provides virtual support to our in-clinic and virtual doctors and advanced practice providers by reducing administrative burden providing effective intake management supporting virtual rooming/end of visit transitions provide triage services to patients support the clinical needs through patient care and in basket management support. This role is part of the Patient Access Team with Dignity Health Medical Foundation.
To be successful in this role you must:
- Use protocol-driven virtual standing orders to perform comprehensive support to physicians and APPs; consistent with RN scope of practice performing at the top of licensure e.g. evaluating lab data providing instruction and education to patients phone triage and medication continuity / modifications
- Give instructions to patients in providing patient vitals and record other information such as blood pressure body temperature heart rate and current medications virtually
- Communicate any patient concerns to physicians and/or advanced practice providers when escalation is needed
- Provide coordination of care which may include facilitating diagnostic and laboratory testing ensuring timely scheduling of follow-up appointments with providers and providing disease management education
- Work with other registered nurses across our locations to identify process improvements and strengthen communication in support of ensuring patient needs are being met
- Provides care support asynchronously e.g. secure messaging with patients via the portal such as addressing non-urgent concerns and providing advice within scope of practice
- Support the Virtual Medical Assistant care members when needed

Your remote workspace must be a HIPPA-compliant environment with physical safeguards to protect health information in your home office.
In this virtual role you may be requested to work from our System office located in Rancho Cordova CA within varying times for requested team meetings. Hours of operation may vary and your ability to work a varied shift that may include weekend hours or select weekends is critical.

Qualifications
Minimum Qualifications:
- Five (5) years of clinical patient care experience as an RN in a patient centered setting. A minimum of one (1) year in the last 5 years as an RN in an ambulatory community involving care management and/or chronic disease management (including outpatient community health clinics private physician offices home health and hospices public health settings free-standing urgent care clinics) or primary care setting.
- Graduate from an accredited school of nursing required.
- Clear and current CA Registered Nurse (RN) license.
- Current BLS through American Heart Association
Preferred Qualifications:
- Experience in care coordination and chronic disease management preferred
- Bachelor of Science in Nursing or other advanced clinical degree preferred.
- CCM Preferred/Desired
- Knowledge of community resources preferred. Familiarity with an electronic practice management system if preferred.
