Program Description

This is an exciting opportunity to be a part of Metro Boston’s first ever PACT-Y team through the Home for Little Wanderers.  PACT  (Program for Assertive Community Treatment) is an evidence-based home/community centered model supported by the Substance Abuse and Mental Health Services Administration (SAMHSA). The Department of Mental Health is launching PACT-Y for the first time for youth and young adults in the Metro Boston area.  PACT-Y utilizes a highly integrated multidisciplinary team model that includes Clinicians, Outreach Counselors, Nursing, Occupational Therapy, Family Partners, Young Adult Peer Mentor, Vocational Specialists, and Psychiatrists/Nurse Prescribers to help support growth and recovery for youth and young adults in the community. Team members provide customized support to youth and young adults based on individual goals. This is your chance to be a part of this incredible new addition to the Massachusetts behavioral health landscape for youth and young adults!

How You Will Be Making A Difference

Responsible for integrating educational and/or vocational goals and services with the tasks of all team members. This staff member provides needed assistance through all phases of the vocational service.
