The Vulnerable Adult / Developmental Disabilities (VA/DD) Targeted Case Management (TCM) Coordinator will provide leadership and oversight of the exploration and development of a Tribal VA/DD Targeted Case management policy, strategic planning and operational work. This includes highly complex program and administrative duties in statewide planning, implementation, and assessment of performance across multiple programs. Responsibilities include:
+ Provide leadership in developing legislation, policy strategies, and compliance activities, with extensive freedom to act in introducing and maintaining services
+ Set strategic direction to meet goals and state and federal requirements
+ Serve as the Transitions, Tribal and Transformations Division’s representative and lead in the research of development and implementation of DHS’ Tribal VA/DD TCM Medical Assistance Benefit efforts
+ Coordinate and conduct Tribal engagement with Minnesota’s 11 federally-recognized Tribal Nations and urban American Indian providers and leaders to design and recommend a Tribal-specific vulnerable adult and developmental disability medical assistance targeted case management benefit
This complex work calls for strong collaboration to consolidate multiple program activities to ensure efficient and effective administration and implementation.
The Tribal VA/DD-TCM Coordinator will represent the division and advise others on the engagement process, service design and reimbursement methodology with Tribal Nations. Additionally, they will be responsible to develop strategic plans to help system partners build and sustain a Tribal specific vulnerable adult and developmental disability medical assistance targeted case management design at the state and local level.
**Minimum Qualifications**
3 years of specialized professional experience working with vulnerable adults and persons with developmental disabilities. Demonstration of experience with the following:
+ Tribal governance and sovereignty, as well as the unique relationship that Tribal Nations and American Indians have with the state and federal government
+ State and local human service delivery systems and funding streams such as for older adults, child welfare, adult and children with disabilities, or behavioral health
+ VA/DD Targeted Case Management, Medical Assistance and home and community-based services and programs
+ Legislative processes, procedures, and protocols
**Preferred Qualifications**
+ Collaborative relationships across a broad and diverse range of internal and external stakeholders
+ Demonstrate leadership and negotiation skills sufficient to build commitment, collaboration, and to resolve conflicts
+ Oversee/write reports, memoranda, and white papers that communicate clear technical subjects to diverse audiences; understand, analyze, interpret, and organize statistical, narrative, and visual data
+ Variety of experiences working effectively with others from different backgrounds and/or cultures
