Location: Chesterfield/Amber Valley
Shift Pattern: 37.5 hours, Monday-Friday, 9am-5pm
Essential: You must hold a valid UK Driving Licence to be considered for this position Vehicle Usage: Use of pool car with fuel card on Outreach / when using own car 45p per mile

What's the Role? As a Wellbeing Worker, you'll offer one-on-one support to individuals who are experiencing mental health difficulties, whether they are self-funded or receiving a personal budget or direct payment.
You'll be their guide, motivator, and advocate, helping them take charge of their lives and achieve their dreams! You'll also collaborate with social workers, mental health professionals, and local services to ensure they receive the full support they need.
Key Responsibilities: Provide tailored support to individuals based on their unique care and support plans
Collaborate with mental health professionals and social workers to ensure comprehensive care
Empower individuals to achieve their goals and aspirations by offering communication, motivation, and coaching
Monitor and adjust support as necessary based on the evolving needs of those you're supporting
Encourage participation in local community services and promote physical and mental health management
Keep records of interactions and report on progress to help continuously improve service delivery
