Position Summary / Career Interest:We are seeking a Workers Compensation/RetainWorks Clinical Nurse Coordinator. This nurse will identify patients to participate in the project, explain the protocols to participate in the project and enroll the patient. This is an exciting new project working with the Public Workforce System.
Below are some of the job functions for this role:
 Schedule follow up appointments/phone calls for the patient
 Capture and document project data
 Work closely with the providers and patients to guide them through the project and activity milestones
 Oversee the day-to-day operations of the RETAINWORKS project
 Coordinating patient care plans by liaising with all necessary care providers and employers to ensure patient needs are met
 Use computer software to coordinate follow up appointments and phone calls
 Work to create efficient systems and workflows within the clinic(s)

Responsibilities and Essential Job Functions

Responsible for management of patient flow, triage, and direct patient care.Liaison of communication between front desk staff, nursing staff, physicians, and clinic managerManage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.Provide nursing support/coordination to the practice to optimize patient outcomes and to improve patient’s quality of life through clinical and educational programs and/or support services.Support patient self-management of disease.Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.Manage high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting, including a registry.Develop relationships with patients and competently uses assessment skills to identify physical, psychosocial, financial, and environmental health care needs of the patient.Educate the patient and appropriate ancillary personnel about the health care needs, safety issues and benefits which include patient education.Utilize available electronic tools, such as EMR, to efficiently document assessments, clinical finds, plans of treatment and progress toward goals.Observes, records and reports patient's condition and reaction to drugs and treatments to physicians. Educates patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.Maintains/reviews patient records, charts and other pertinent information. Records tests and examination results and then follows up with patient.  Posts tests and examination results.Responds to/refers incoming patient treatment-related phone calls.  Instructs patient and family regarding medications and treatment instructions.Maintains patient confidentiality.In providing patient care, document using EMR to maintain and/or develop patient records in compliance with practice standardsMust be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities. Required Education and Experience

High School GraduateBachelor Degree Nursing2 or more years of RN experience


Preferred Education and Experience

Previous medical office experience


Required Licensure and Certification

Basic Life Support Heartsaver (BLS HS) (Non-Clinical) - American Heart Association (AHA) or obtained within 14 daysLicensed Registered Nurse (LRN) - Multi-State - State Board of Nursing


Preferred Licensure and Certification

Must have current active multi-state Registered Nurse (RN) license in good standing in state in which you live OR active single state RN license in good standing in the state you will be working, with proof of application of a multi-state RN license in the state in which you live.

Staff hired prior to 2/1/2024 are required to obtain a multi-state RN license from the state in which they live at the time of their next RN license renewal by 3/31/2026.
