**Job Purpose Statement:** The Wound, Ostomy, Continence (WOCN) Clinical Nurse Educator is a clinical leader who is responsible for prevention, planning and providing care for patients with complicated wounds, ostomies, fistulas, pressure sores and incontinence nursing care at UCSF Benioff Children's Hospital Oakland (BCH Oakland). _The advanced knowledge and skills required for this role include clinical expertise in a focus area, evidence-based practice, education, consultation, and leadership._ The position works in collaboration with the Nursing Leadership, Faculty/providers, APNs, and Nursing Staff at BCH Oakland. The WOCN will be responsible for the development of a comprehensive program for the prevention, care, education, and rehabilitation of children requiring wound, ostomy, and continence management. The WOCN will perform initial and ongoing patient assessments and receive consult requests. The WOCN will collaborate to formulate individualized treatment plans based on the needs of the patient and family, and oversee implementation of the treatment plan. The WOCN will refine or develop as necessary patient education tools and materials for APEX that will facilitate the education of patients/families in the management of wounds, ostomies, tubes, drains, and wound vacs. The WOCN will document face-to-face encounters with clinical notes in the medical record. Treatment plans will be coordinated with the UCSF discharge coordinators to better meet the needs of the patients, families, and referring providers after discharge. The WOCN may communicate with referring providers at the time of admission and discharge and will coordinate community follow-up to facilitate a successful transition. The WOCN will work collaboratively with an interdisciplinary team that includes nurses, physicians, administrative staff, as well as allied health professionals to provide consistent care to each patient/family. The WOCN will assess the needs of the nursing and medical staff and provide educational events to meet these needs initially and ongoing. **Essential Functions:** + **Patient Care Consultation** + Serves as a reliable source of information on the latest evidence supporting cost-effective, safe nursing practices. + Conducts comprehensive, holistic wellness and illness assessments using established or innovative evidence-based techniques, tools, and methods. + Collaborates with the multidisciplinary team using the nursing process to integrate the nursing perspective into a comprehensive plan of care for the patient/family. + Identifies and prioritizes nursing care needs for a select population of patients/families. + Evaluates the impact of nursing plan of care on fiscal and human resources + Initiates and plans care conferences or programs for individual patients or populations of patients. + Designs and evaluates innovative educational programs for patients, families, and groups. + Identifies, collects, and analyzes data that serve as a basis for program design and outcome measurement. + Establishes methods to evaluate and document nursing interventions. + **Nursing Practice** + Collaborates with others to resolve issues related to patient care, communication, policies, and resources. + Collaborates to revise nursing procedures using evidence-based information to achieve outcomes for indicators that are nurse-sensitive. + Identifies facilitators and addresses barriers that affect patient outcomes. + Participates in clinical practice and quality improvement initiatives for a unit or a program. + Collaborates with nurses to develop practice environments that support shared decision-making. + Assists the staff in developing critical thinking and clinical judgment. + Fosters a nursing care environment that stimulates continuous self-learning, reflective practice, feeling of ownership, and demonstration of responsibility and accountability. + Collaborates with Clinical Nurse Educators and others on clinical competencies, and other clinical educational program development. + Mentors nurses to acquire new skills, develop their careers, and effectively incorporate evidence into practice. + Provides input for staff evaluation. + Provides formal and informal education for nurses and other health professionals and health professional students. + Collaborates with the Nurse Scientists and CNSs and participates in the conduct and utilization of nursing research. + Partners with nurse leaders to disseminate practice changes and provides training on demand as needed to meet or exceed appropriate professional organization or regulatory standards. + **Organizational and System Leadership** + Consults with other units and health care professionals to improve care. + Assists institutional groups to enhance the clinical practice of nurses and improve patient outcomes. + Participates in the development, piloting, and evaluation of innovative practice models across the continuum of care. + Participates in the design and evaluation of programs that are congruent with the organization's strategic plans, regulatory agency requirements, and nursing standards. + Participates in need identification, selection and evaluation of clinical/medical products and equipment. + Participates in committees, work groups and/or projects related to clinical practice. + Serves in professional organizations and contributes to the literature and professional community through posters, presentations and publications. + Increases the visibility of UCSF Medical Center and UCSF Benioff Children¿s Hospital Oakland at local, regional, national and international levels. + Responsible for the written document for Magnet submission Knowledge** + In-depth knowledge of one or more clinical specialty areas, including diagnoses, treatments, standards of practice, and patient/family educational consulting and resources. + Advanced critical thinking and problem-solving skills to assess highly complex issues and develop multiple options for resolution. + Advanced skills in written and verbal communications, with the ability to convey complex and/or sensitive information in a clear and concise manner. + Advanced interpersonal skills, with the ability to provide education, training, and direction to nursing staff, and to develop positive working relationships with patients, families, employees, and outside agencies as required. + Flexibility to orient and work at all UCSF Medical Center locations **Minimum Education:** + Bachelor¿s degree from an accredited Nursing program required. + Bachelor¿s degree or master¿s entry to practice degree from an accredited Nursing program required. + Master¿s degree from an accredited Nursing program preferred. **Minimum Experience:** + Minimum of two (2) years of recent clinical RN experience in the specialty area required. **Additional Minimum Requirements:** + Licensed as a Registered Nurse in the State of California required + California APRN license preferred. + Current American Heart Association CPR Certification required + WOCN Certification required.Certification may be progress with completion within 1 year of hire. **Physical Requirements:** + Sit: Up to 3 hours/day + Stand/Walk: 6-8 hours + Bend/Stoop: Up to 3 hours + Reach: Up to 6
