Requirements
Education: Graduate of an accredited school of nursing; BSN preferred
Licensure: Current and valid Registered Nurse (RN) license in the state of Pennsylvania
Certification: Certified Wound Ostomy Continence Nurse (CWOCN) certification required
Experience: Minimum of two years of experience as a Registered Nurse in a relevant clinical setting; prior experience in wound care, ostomy care, and/or continence management preferred
Skills: Excellent assessment, critical thinking, and problem-solving skills; strong interpersonal and communication skills; proficiency in electronic medical records systems; ability to work independently and as part of a team; current CPR/BLS certification; valid driver's license and reliable transportation
Position Details
Responsibilities:
Conduct comprehensive assessments of patients' wounds, ostomies, and continence issues
Develop individualized plans of care in collaboration with physicians and interdisciplinary teams
Select appropriate wound dressings, therapies, and equipment based on evidence-based practice guidelines
Educate patients and families on wound care techniques, ostomy care, and continence management strategies
Provide direct patient care, including wound dressing changes, ostomy care, catheter care, and skin care
Monitor patient progress and adjust care plans as needed
Provide emotional and psychosocial support to patients and families
Participate in quality improvement activities related to wound care, ostomy care, and continence management
Maintain accurate and timely documentation of all patient assessments, interventions, and outcomes
