Job Summary:

The Nurse Practitioner is a leadership role within a multi\disciplinary and inter-professional team. This Nurse Practitioner role in NLOT team is primarily responsible for providing wound care clinical support Long Term Care (LTC) homes. The NP works in collaboration and consultation with a physician, the LTC or congregate setting coordinators, and members of the inter-professional health care team to support complex wound care in the LTC and prevent ED visits. The NP will act as a link between hospital and the LTC homes to ensure the most appropriate and efficient care is received in relation to their wounds.

 

Responsibilities:

 

Provision of Nurse Practitioner scope of clinical care for LTC residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled actsCollaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissionsCapacity building for LTC to develop and enhance competency for acute and chronic woundsIntegration of best practice guidelines and current evidence-based research into clinical practice for a select patient populationEvaluation of outcomes for the programs and make recommendations for change in consultation with Program Director and ManagerDevelopment of linkages with external partners as appropriate to ensure continuity of careEnhance communication between hospital and LTC to support the plan of careIn keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker.Utilization of expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client.Participate in meetings and committees to support the planning and implementation of best practices to support needs of the residents in LTC homesParticipates in reflective practice as per identified college standards

Requirements:

Master in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies requiredCurrent College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required1-3 years of demonstrated experience as a Nurse Practitioner requiredPrevious experience in LTC sector preferredMinimum 2-3 years direct clinical practice in complex wound management required

- Academic and clinical preparation in complex wound care management from a recognized education program an asset

CNA Certification in Geriatrics would be an assetBLS requiredAdditional specialty education in Gerontology preferredDemonstrated experience in Adult education an assetClinical specialty organizational membership: RNAO (required), CANO or ONS also preferredDemonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholdersPossesses highly developed analytical, problem solving and critical thinking skillsExcellent oral, written, and presentation skills, with demonstrated competency in information and communication technologyDemonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demandsDemonstrates an ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situationsExperience in Case Management preferred.Embraces a philosophy of patient and family centered care.Demonstrates leadership in clinical excellence.Assesses and provides services to clients, families and communities.Provides comprehensive health services encompassingIdentification and management of chronic stable conditions.Prevention of disease and injuries.Health promotion and education.Continuity of careAdvanced nursing assessment skills and sound knowledge of clinical therapeutics.Effective patient/family counseling skills.Excellent organizational, critical appraisal and analytical skills.Ability to work independently and as part of a team.High level of initiative and self-direction.Excellent attendance and a discipline free record is required (candidates are required to inform HR and the hiring manager of their attendance and discipline status)Vehicle and valid Ontario driver’s license required
