Employment Type:Full timeShift:Description:Purpose: Collaborates with the management team to integrate evidence based practice at the bedside with RN's. Teach the RN's communication and customer service skills, assess their clinical skills and educate as needed. Be a mentor and help coach them during concerns and offer career development.
What you will do:
Utilizes clinical skills and knowledge in the assessment of clients' health status assessment of clients' health status (physiologic, psychosocial, emotional, cultural, and spiritual)Conducts initial assessment and reassessment of patient/family's WOC needs and develops a specific plan of care to meet those needs with the bedside nurseProvides consultation and/or assessment on patients with pressure and non-stageable pressure injuriesIn consultation with the physician, assists the primary nurse in developing an appropriate plan of care for comprehensive wound managementMakes recommendations to the physician for changes to wound care orders and provides evidence-based research support as neededCorrelate assessment findings with appropriate interventions and follow-upPerforms and documents comprehensive wound care assessment upon each treatmentUses comprehensive understanding of wound, ostomy and continence anatomy and physiology and continuum of care defined by the national WOC nursing standardsRecommends and develops protocols and clinical pathways to ensure quality WOC care, including prevention of pressure injuriesIntermittently works with medical staff, Nurse Managers, Professional Practice, nursing staff, dietitians, and case managers in multiple hospitals to facilitate and provide basic WOC care and education for inpatientsCollaborate with nursing leaders to assure integration of services.Assist with care coordination and provide hands on care to patients as requestedInvestigates all cases with adverse events related to wounds as requestedReviews charts of patients with adverse events related to wounds and ostomy as requestedProvide education to staff and consultation for patients and their families who are undergoing ostomy surgery, and patients with pre-existing stomas who are having trouble managing their care.Consult, assess, and plan initial nursing care plan for patient with post-operative stomas and/or altered skin integrity and provide ongoing support and evaluationParticipates in clinical outcome monitoring, follow-up, and performance improvement initiativesConduct quarterly pressure injury prevalence surveys with unit skin champions. Report metrics to quality, professional practice team and nursing leadership and provide ways to offer improvementSubmit data as requested by Quality Director as part of the Board reportWorking knowledge of quality management and resource utilization methodologiesComputer skills in Microsoft office and be adaptable to new software programs; EPIC EMR experience is a plusAbility to complete concise, thorough clinical documentation of patient assessments and carePrevious educator, leadership/preceptor experience strongly preferredParticipates in evaluation of products and new technology for effectiveness and cost to ensure the most cost-effective products are used for patient care.Minimum qualifications
BSN required; MSN preferredMinimum of 3 years of licensed professional nursing experience in acute care. Minimum of 2 year of experience in wound care/ ostomy management.Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations.Must know principles of adult learnersMust be able to communicate articulately and comprehend written and verbal communications in English.Must be able to function and prioritize well within a dynamic work environment.Expected Behaviors: Customer service, diplomacy; interpersonal skills; maintain confidentiality; ability to work and communicate effectively with diverse group of people; adherence to established timelines; and professionalism.Licensure/Certifications
