Wound Care RN - Wound Care Clinic
The hours for this position are 8:30am -5:00pm The Wound Care Registered Nurse (RN) is primarily responsible for providing direct and indirect patient care to the Wound Care Center patients as scheduled under the supervision of a group of providers. Must be able to use critical thinking and leadership skills to prioritize needs of clinic, allowing for most efficient clinic flow, and optimal patient experience. The RN will demonstrate knowledge of the nursing process as a standard, incorporating elements of holistic patient centered care through the collection, verification and organization of data about a patient’s health status, planning for care, implementation of the plan and evaluation. The incumbent will be required to function independently during Nurse Appointments, which include but are not limited to physical assessments and pain assessment and documentation in the electronic health record. This individual will possess strong assessment skills, be able to multitask and react to changing situations easily. Must assure that patients are appropriately educated and scheduled for procedures as necessary. Assist and train staff to support physician in-office procedures. Required to accommodate for and attend 80% of all scheduled meetings. All staff is considered first to be an employee of St. Peter’s Medical Group first. Because of this, there is an expectation that they may float to other SPHMG areas when not needed in assigned area. Duties include, but are not limited to the following: + Systematic collection and assessment of data in collaboration with the multidisciplinary team to provide therapeutic care for the patient and/or family. + Intake of patient, verification and recording of pertinent clinical data and health data, clinical quality measures, update medical record, administer appropriate screening tools, while providing excellent patient care + Perform hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics hyperbaric training. + Documents appropriate correspondence, assessments and care in patient’s EMR. + Acts as a resource and mentor for new and inexperienced nurses as needed. + Able to prioritize work load, respond to patient calls and messages in accordance with policy + Guide patients through the health care system and help patients arrive at scheduled appointments on time and prepared. + Provide emotional support related to the patient’s clinical situation in accordance with the patient’s plan of care. + Serve as a patient advocate and assist patients in navigation along the care continuum with the goal of facilitating effective quality care and timeliness, resulting in improved patient satisfaction and outcomes. + Assist with referrals of patients to the appropriate specialist for diagnosis or treatment. + Maintains open lines of communication with the patient, physician, co-workers, and wound center medical director about changes in a patient's clinical condition including results of lab studies and symptomology. + Is able to use critical thinking skills and respond quickly and accurately to changes in condition or response to treatment + Other duties as assigned.
