Responsibilities

We are seeking a highly motivated and compassionate individual to join our team as a Wraparound Facilitator. As a Wraparound Facilitator, you will play a crucial role in empowering and supporting individuals and families in need, ensuring they receive comprehensive and personalized services. If you are passionate about making a positive impact in the lives of others, possess exceptional communication and problem-solving skills, and thrive in a collaborative and dynamic environment, we invite you to become a part of our dedicated team and help foster meaningful change within our community.

What will I be doing?

Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.Develop service plans with the input of the treatment team and the Clinical SupervisorConnect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.Complete documentation within established time framesMeet with clients weekly or bi-weekly for up to 12 months.Attend and participate bi-weekly with Treatment Team meetingsServe as a liaison with other professionals and agencies as related to the clientActively recruit new referrals to ensure that your caseload stays fullProvide 24/7 on-call support for clients as per program requirements Qualifications Bachelor’s Degree in human services discipline, including, but not limited to: Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc.).Experience working with children and families in case management type/ community resource position.Must be certified by DMH as a CSS.  Options ApplyApplySubmit a ReferralRefer Sorry the Share function is not working properly at this moment.
