We have an exciting opportunity for a Young Persons Transitional Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.
The post holder will engage young people in a range of interventions, utilizing strengths based and young person-centered approaches, empowering them to reduce risk, increase resilience and lead a safe, happy, and healthy life.
No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

Full Time Hours: 37.5hrs per week
Full Time Salary: £27,449 - £31,529 dependent on experience
Allowance: Outer London Weighting of £2163.27 pro rata
ResponsibilitiesAbout the role:
Engage young people who have substance use issues and provide them with advice, support, and structured interventions to support their health and wellbeing needsWorking with clients between the age of 18 and 25 supporting the transition between young people’s services and adult services.Work and liaise with specific young adults agencies involved with the young adult’s broader care plan.Complete comprehensive screening and assessments and develop individual care plans that incorporate integrated care and transition supportPromoting young adults and young people’s participation and co-production opportunities to support identification of potential gaps in provision and unmet needsWork closely within the 16+ supported accommodation Multi-Disciplinary Team model to provide a co-ordinated, flexible and responsive approach to support
About you:
Knowledge and experience of providing interventions for service users affected by Substance use.Be an excellent communicator, both verbal and written with good IT skills.Proactive and able to work on own initiative with excellent time management and prioritising skills.Have a good working knowledge of mental health interventions, services, and good practice.Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.Ability to manage change successfully in a way that prioritises the needs of service users.Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.What we will give to you:
