Youth Rapid Rehousing Case Manager & Assistant Housing Program Coordinator
Overview Location: Waterbury Housing Services (Waterbury, CT) Hours Per Week: 40 (on-call responsibility for two weeks of every four weeks, weekend hours as needed) Schedule: Monday - Friday, 8:00 am – 4:00 pm; may vary depending on need Scope & Purpose of Position: To support and guide youth, ages 18-24, to safe, secure, and permanent housing; responsible for supporting and guiding families and young adults to secure permanent housing. Assist in The Salvation Army’s homeless services programs in the Waterbury area, including staff, program planning, implementation, and evaluation. Responsibilities + Assist in program planning, evaluation, and reporting. + Be knowledgeable of all HUD definitions and requirements for ESG and CT RRH assistance. + Complete statistical reports . + Maintain accurate and up-to-date files and records. + Maintain awareness of facility and safety needs and report concerns. + Develop and maintain an updated list of available units for youth and families; inspect housing units using HUD quality standards as guidelines. + Promote and ensure trauma-informed care. + Provide crisis intervention as needed. + Regularly evaluate and assess all program components to determine effectiveness in achieving goals. Youth Rapid Rehousing Case Manager + Assist youth leaving Waterbury shelters in locating decent, affordable housing. + Identify, screen, and enroll eligible individuals. + Get referrals from the housing placement committee directly from the Coordinated Access Network (CAN). + Visit all shelters within the CAN geographic area regularly for screening and outreach to youth. + Develop a Housing Service Plan with enrolled youth within three working days of initial enrollment and ensure implementation. + Provide ongoing intense case management to the youth accepted into the program for the duration of their enrollment. + Network with human services agencies to coordinate support to enrolled youth. + Assist youth in registering for and securing placement in training classes of their choice regarding employment and education. + Develop and implement a monthly schedule for outside resources to encourage and educate youth on specific community resources. + Educate youth and landlords on tenant/landlord rights and responsibilities and facilitate the relationship between tenants and landlords. + Establish rapport with landlords and the Housing Authority to discuss landlord support needs and concerns, unit availability, etc. + Provide support and follow-up services to youth leaving the shelters and their landlords to establish and maintain rental stability. + Survey youth and landlords to measure satisfaction with housing support. + Visit youth in their homes for support of housing stability. + Network with CT ESG Provider and CT Coalition to End Homelessness to request financial assistance to eligible clients for RRH services and coordinate handling third-party payments to vendors. + Attend and participate in meetings, including staff meetings, as requested or scheduled by the supervisor or the Corps Officers. Assistant Housing Program Coordinator + Assist with managing a low-barrier, housing-focused shelter for unsheltered families. + In the absence of the Housing Program Coordinator, provide oversight to the shelter, staff, and general operations. + Train and coach case management staff to be client-centered and utilize appropriate evidence-based techniques when working with shelter clients. + Ensure compliance with local, state, and federal guidelines, including the Northwest CAN policies. + Be on-call for staff emergencies. + Identify, screen, and enroll eligible families in programs as appropriate. Qualifications + Bachelor’s Degree or a minimum of two years of experience in a related field is preferred. + Ability to function independently, with excellent time management skills. + Ability to respond to changing conditions. + Bilingual in English and Spanish is preferred. + Communicate both verbally and in writing. + Demonstrate excellent ability to engage with diverse populations. + Good interpersonal, organizational, and oral/written communication skills. + High energy level and solid work ethic. + Knowledge and proficiency in Microsoft Office programs, particularly Excel and Word. + Must complete KeepSAfe and other training requirements upon hire. + Must have a valid Connecticut driver’s license. + Work well in a team atmosphere. + Will respect and promote the interest of The Salvation Army.
