Provide case management services to the residents of ZG Family Emergency Shelter.

 

 

 

This position will pay $20.00/hr

Responsibilities Provide direct service to all assigned clients consistent with ethical social work practice as described in the NASW Code of Ethics.Comply with all the requirements for licensure in the State of Ohio Counselor and Social Worker Board as outlined in the Ohio Revised Code.Screen for appropriateness of services. Conduct intakes in compliance with McKenny Vento’s definition of homelessness and HUD’s definition of chronic homelessness on an as-needed basis; review shelter policies and procedures.Assess client’s needs and employability. Complete and assure accuracy of all required forms, including but not limited to United Way, HUD and Self Determination forms.Follow current admission policy regarding new admits and transferring of clients to other rooms as it pertains to hygiene and handling of clothing.Develop an Individual Service Plan (ISP) with the client. Refer clients to appropriate mainstream services.  Provide on-going assessment and services.Provide weekly counseling and other supportive services as needed.Maintain timely progress notes on all services provided. Progress notes are to be completed at or close to the time of service.Assure confidentiality of client information and records, except when failure to disclose would be a violation of the law. Obtain releases of information from the client when sharing information.Refer clients to Mental Health Services (216-623-6888) if they present as a risk to themselves or others. Refer clients to Children and Family Services (216-696-KIDS) when there is suspected abuse or neglect of children.Conduct at least one group meeting per week with clients based on HUD agreement.Resolve all issues in person concerning clients as indicated on the incident reports or special reports.Assure safety of all residents and staff by contacting Security and/or 911 in emergencies. Assist with de-escalation when needed, especially if it involves your assigned client.  Implement the Facility Emergency Evacuation Procedure (FEEP) as required.Issue violation points; bring severe behavioral issues and potential discharges to the Program Manger’s attention to help problem solve.Attend all mandatory meetings and trainings.Perform other duties as assigned by the Program Manager or Executive Director. Qualifications

Associate/Bachelor degree in Social Service field required for a case manager.

Previous experience working with children and families preferred.Knowledge and compliance with the NASW Code of Ethics.Knowledge of community resources.Must be able to routinely lift and carry boxes weighing 50 pounds or more.Must be able to use stairs, when necessary, for building evacuations, drills or during elevator maintenance.Good assessment and strong judgment skills.Good communication skills including good grammar, spelling and legible handwriting.Computer literate.
